What to Bring:

For your first visit, please bring the following items with you:

  1. Completed patient intake form. After you make your appointment, I will email the form to you.
  2. Please bring copies of any lab work that you've had recently.
  3. Please make a list of all supplements and medications that you take regularly. The more we know about what you're already taking, the better we can serve you.
  4. Please wear comfortable, loose fitting clothing. Shorts and T-shirt are great or clothes that can easily be pulled up past the knees and elbows without cutting off circulation.

    If you're not sure if you have acupuncture coverage with your insurance, feel free to call or fax your information to us and we'll verify it for you. Typically, HMO's do not cover acupuncture. Lots of PPO's do cover acupuncture.
     
    Every insurance policy varies, so please Fax or email the following information to FAX # 949-305-1773
    • Copies of the front and back sides of your insurance card (please make sure that the numbers can be read)
    • Your date of birth
    • Name of primary card holder
    • Your name (since it may be a spouse or parents name on the card)
    • Phone # where you can be reached
    • Medical reason for your visit
    • Your email address
